This questionnaire was developed from my own extensive, scientific research. You'll find it nowhere else.
It gives you tools to assist you in obtaining a certain degree of stress relief so that the dis-ease you feel emotionally, physically and spiritually can be lessened.

Notice I said dis-ease. Disease is dis-ease of the body and metaphysically there are reasons certain diseases settle in various parts of our bodies. When stress is lessened and you make the mind/body connection, your body can condition itself to self heal, possibly not 100% but enough to make you more comfortable.
I'm not saying to stop seeing your doctor or stop taking medication. I am not a medical doctor, so no claims of a "cure" are being made. My process will enhance your taking charge of your life and aid in your commitment to yourself to self heal. I will educate you in Health & Life Science and give information from which to choose! This process fosters physical, emotional, social and spiritual growth.
Your questionnaire is hand scored, not computer generated, and the reports are hand written & created and individualized. I do not use a pre-made scoring template. I spend at least 20 hours on each one.

There are over 425 items in my 18-page questionnaire about your physical, emotional, social and spiritual condition and concerns. Most of the questions are a check off, SOMETIMES, NEVER, OFTEN. Some are explanations you need to write.
Take your time, do not rush through it. It is a copious amount to do, and I do not impose a deadline.

1) Download the questionnaire in two parts

The downloaded questionnaire comes in two parts.
- Part One
As soon as you pay you collect Part One. This contains the RELEASE FORM (please appreciate that I won't do the analysis without it signed by you) and narrative questions which I'd like you to explain in as much detail as you wish. There are six pages, and the last one is for any additional notes. - Part Two
Five days later, you'll receive Part Two. This has the rest of the questionnaire — fourteen pages of detailed questions. (Almost all of them are a check off.)
The reasons I split the download questionnaire this way is to let your credit card clear, and to give you time to answer the narrative questions.

After I receive your payment (which can be with a money order or major credit card, but not a personal check) I will post you by snail-mail the Holistic Healing Questionnaire and my standard Release Form.
